Nomination Forms for 2023
Thank you for recognizing the leaders in philanthropy in Story County. We are proud to be able to have great members of the community and want to make sure they are recognized. Thank you for taking the time to show your appreciation for all that these special people do for our community by nominating them for a Story County Alliance for Philanthropy award. The awards will be presented on Philanthropy Day, November 16, 2023 at the Gateway Hotel and Conference Center in Ames.
The appropriate nomination forms will be mailed to those who submitted an Intent to Nominate form after June 16 or you can find them below.
These forms are for the 2023 nomination process and are outdated. They are included as examples of the type of information needed as you consider your nomination for 2024.
